Benefit Dependent Audit Verification
All benefits-eligible employees must furnish documentation of dependent(s) eligibility. Please see Memorandum and FAQ for details.
Employees with no dependents are not required to participate in the Dependent Audit Verification; however, participation is highly recommended.
During your meeting, you will have an opportunity to voluntarily discuss other insurance programs that may be of interest to you. You are not obligated to review or purchase any of the available insurance plans. Please also see FAQ below.
What about tax return documents for same sex spouses?
The IRS does not permit same sex spouses to check the "married" box or to file a joint federal tax return. Accordingly, for those who have a same sex spouse, a copy of the 2010 California joint state tax return along with an original or certified copy of the Marriage Certificate will be accepted in place of the Federal IRS tax return document
Marriage Certificates:
If an employee does not already have an original marriage certificate--- all three types "informational," the "authorized/confidential," or the "authorized/public" are acceptable for the purposes of this audit, along with the first two pages of your most recent (2010 or 2011) IRS tax return document.
The quickest way to obtain a marriage certificate is by requesting it from the county recorder in the county where the marriage took place. Informational certificates may also be quicker to obtain since a notarized statement is not needed."
The CA Department of Public Health (cdph.ca.gov) website suggests requesting the marriage record from the County Recorder's Office in the county where the marriage license was issued and there is a link to a list of County Recorders:

The County Recorder processes most mail requests within 20 working days. Requests may also be made over the internet with a credit card and have a faster turnaround time.
Employees in need of marriage certificates must complete a notarized "Application for Marriage Record" form.

Marriage certificates versus church certificates?
In the recent weeks, there have been several questions regarding marriage certificates and church certificates for the dependent audit verification. Church certificates will not be accepted for verifying a marriage. A certified copy of a marriage certificate issued by the County Clerk Recorder is required. However, if you do not have a certified copy of a marriage certificate, a marriage certificate with an embossed seal (not a State watermark seal) that you may already have will be accepted. If your embossed seal is not legible, you will be asked by the auditor to bring a certified copy marriage certificate issued by the County Clerk Recorder.
