Form Sample - Employee Tuition Reimbursement

Sample scenario:

Jasmine Doe works in the Business Services department at College A. She was interested in learning more about what drives her local economy and, in turn, her community college. She registered for a Microeconomics course at the college and passed with flying colors. At the end of the semester, Jasmine submitted her transcript and completed Enrollment Fee Reimbursement Application to District Business Services.

This form should be submitted immediately after the course is completed because this program is administered on a first come, first served basis. Per SEIU Local 1021 bargaining unit agreement Article 15.9, reimbursement is limited to $750 per employee per fiscal year and $9,000 total for the fiscal year.
